Date of admission: 31-10-2022 CHIEF COMPLAINT: patient came to the opd with complaints of fever since 3 days and c/o cough associated with sputum  since 3 days  C/o hoarseness of voice and throat pain since 3 days  C/o decreased appetite since 5-6 months  C/o weight loss approximately 20kg over the last 1 year. HISTORY OF PRESENT ILLNESS- Patient was apparently asymptomatic 3 days back then she got fever and cough associated with sputum. She also had hoarseness of voice since 3 days.
